In the SURMOUNT-5 trial, only 2.7% of tirzepatide patients and 5.6% of semaglutide patients discontinued treatment due to GI side effects. That means over 94% of people tolerated them well enough to continue treatment for the full 72 weeks.
- Eat smaller, more frequent meals during the first few weeks. Large meals on a GLP-1 are genuinely uncomfortable.
- Avoid greasy, fried, or very rich foods during the adjustment period. These are the most common triggers for nausea.
- Stay hydrated. Aim for at least 64 ounces of water per day.
- Take your injection at a consistent time each week. Some patients find evening injection reduces next-day nausea.
- Get enough protein. Many providers recommend 60–100 grams of protein per day while on GLP-1 treatment.
GLP-1 medications carry a boxed warning about thyroid C-cell tumors, based on animal studies. They should not be used by anyone with:
- • Personal or family history of medullary thyroid carcinoma (MTC)
- • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- • History of pancreatitis or severe gastrointestinal conditions
- • Pregnancy or nursing
