TrimRx Side Effects: What to Expect and When to Get Help
By WPG Research Team · Last verified · Last updated
TrimRx side effects are mostly stomach and gut problems: nausea, diarrhea, constipation, vomiting and stomach pain, often when you start or raise a dose. TrimRx's medicines are compounded and have no side-effect studies of their own. On brand-name versions, nausea hit 25% to 44% of people. Serious problems are rare but real.
Weight Loss Provider Guide is an independent comparison resource for GLP-1 telehealth providers.
The short answer
- Most side effects are gut-related. TrimRx lists nausea, constipation or diarrhea, burping or reflux, stomach cramps, vomiting, tiredness, headache, dizziness and injection-site soreness.
- They're most likely early and after dose increases. TrimRx says side effects can happen when you start or when your dose changes.
- There's no TrimRx-specific data. TrimRx's main program uses compounded semaglutide and tirzepatide, which are not FDA-approved. The best numbers we have come from the brand-name Wegovy and Zepbound labels.
- Some signs need fast help. Severe stomach pain, not keeping fluids down, or swelling of your face or throat are not "normal side effects."
GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.

How common are these side effects?
TrimRx doesn't publish how often its members get side effects. The closest real numbers come from the FDA labels for Wegovy (semaglutide) and Zepbound (tirzepatide). These are the brand-name medicines, not TrimRx's compounded versions.
| Side effect | Wegovy 2.4 mg | Zepbound (5 to 15 mg) | Placebo (no drug) |
|---|---|---|---|
| Nausea | 44% | 25% to 29% | 8% to 16% |
| Diarrhea | 30% | 19% to 23% | 8% to 16% |
| Vomiting | 24% | 8% to 13% | 2% to 6% |
| Constipation | 24% | 11% to 17% | 5% to 11% |
| Stomach pain | 20% | 9% to 10% | 5% to 10% |
| Headache | 14% | Not in the top list | 10% (Wegovy trials) |
| Tiredness | 11% | 5% to 7% | 3% to 5% |
| Indigestion | 9% | 9% to 10% | 3% to 4% |
| Dizziness | 8% | 4% to 5% | 2% to 4% |
| Burping | 7% | 4% to 5% | 1% or less |
| Injection-site reactions | Not in the top list | 6% to 8% | 2% |
| Hair loss | 3% | 4% to 5% | 1% |
Placebo ranges cover both the Zepbound and Wegovy trials.
What this tells you:
- Nausea is the big one. It was the most common side effect on both medicines.
- Many people get through it. In the label trials, 6.8% of people on Wegovy and 4.8% to 6.7% on Zepbound stopped because of side effects. On placebo, 3.2% to 3.4% stopped.
- The early months are the hardest. Most people who stopped Zepbound because of side effects did so in the first few months, because of stomach problems.
Is semaglutide or tirzepatide easier on the stomach?
TrimRx offers both: GLP-1 (compounded semaglutide) and GLP-1 + GIP (compounded tirzepatide). In the brand-name labels, nausea, vomiting, diarrhea and constipation showed up in a larger share of Wegovy patients than Zepbound patients.
But these come from different trials, so it's not a fair head-to-head. And no one has tested TrimRx's compounded versions. Your provider picks the medicine and dose for you. More: semaglutide vs. tirzepatide and tirzepatide side effects.
Serious side effects to know about
These are less common, but the FDA labels for both medicines warn about them. Get medical help if you notice the signs.
| Problem | Signs to watch for |
|---|---|
| Thyroid tumors (boxed warning) | A lump or swelling in your neck, trouble swallowing, a hoarse voice, or shortness of breath |
| Pancreatitis | Severe stomach pain that won't go away, sometimes spreading to your back, with or without vomiting |
| Gallbladder problems | Pain in your upper stomach, fever, yellow skin or eyes, or clay-colored stools |
| Kidney problems from fluid loss | Vomiting or diarrhea that leaves you dehydrated |
| Severe stomach reactions | Stomach problems that are severe or don't let up |
| Serious allergic reaction | Swelling of the face, lips, tongue or throat, trouble breathing, fast heartbeat |
| Low blood sugar | Shakiness, sweating, confusion. Higher risk if you also take insulin or certain diabetes pills |
| Eye problems in type 2 diabetes | Vision changes, if you have diabetic eye disease |
| Problems during surgery or sedation | Food can stay in your stomach longer. Tell any doctor before a procedure |
The Wegovy label also warns about a higher resting heart rate. The Zepbound label says it can make birth control pills work less well. It advises switching to a non-pill method, or adding a barrier method, for 4 weeks after you start and after each dose increase.
TrimRx says to call 911 if you have trouble breathing, swelling of your face, lips, tongue or throat, severe chest pain, or think you're having an emergency.

Are side effects different because TrimRx is compounded?
They may be. Nobody knows for sure, because compounded versions aren't tested the way brand-name drugs are.
- TrimRx says so itself. Its safety page says compounded medicines "may have different side effects or risks than FDA-approved versions," that quality can vary between pharmacies, and that there's limited long-term safety data.
- The FDA doesn't review them. The FDA says compounded drugs are not FDA-approved and it does not review them for safety, effectiveness or quality.
- The FDA has received reports. As of May 31, 2026, it had 990 reports of side effects linked to compounded semaglutide and more than 730 linked to compounded tirzepatide. It says many look like the brand-name side effects, and that the real number is likely higher because many compounding pharmacies don't have to report.
- Dosing mistakes are a known risk. The FDA has had reports, some needing a hospital stay, of people measuring the wrong dose of compounded semaglutide from a vial.
That last point matters at TrimRx, because its medicine comes as a vial you draw up with a syringe. TrimRx says to stop and message your Care Team before injecting if the liquid looks cloudy or has bits in it, if the syringe markings don't seem to match your instructions, or if you're not sure you drew the right amount.

What helps, and when to message your Care Team
TrimRx's own tips for common side effects:
- Eat smaller meals and stop when you feel full.
- Drink fluids through the day.
- Follow the instructions that came with your prescription.
- Never double up. If you miss a dose, TrimRx says not to take more than your prescribed dose to make up for it.
- Don't change your dose on your own. TrimRx says not to change, stop or switch your medicine without your provider.
Message your Care Team in the TrimRx portal if you:
- Have nausea, vomiting, diarrhea or constipation that keeps going
- Can't keep fluids down
- Feel dizzy, weak or dehydrated
- Aren't sure whether to take your next dose

Who should talk to a doctor before TrimRx?
TrimRx asks you to tell your provider if you've had any of these: type 1 or type 2 diabetes, thyroid cancer, stomach or gut disease, pancreatitis, kidney disease, diabetic eye disease, depression, or suicidal thoughts. Also tell them if you're pregnant, trying to get pregnant, or breastfeeding.
GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.
Can you get your money back if you can't handle the side effects?
Maybe not. TrimRx's refund policy says requests must be made within 28 days of your original purchase, and medicine can't be returned once it ships. Canceling stops future charges but doesn't refund a charge that already went through. See how to cancel TrimRx.
That's a good reason to start with a plan length you're comfortable with, and to message your Care Team early if side effects are hard to live with.
Is TrimRx still worth trying?
For most people, the side effects are uncomfortable but manageable, and they tend to ease after the first months. If you're fine with compounded medicine, want to pay cash, and plan to follow your provider's dose schedule, TrimRx can still fit. TrimRx's GLP-1 plan starts at $179 a month. FAQ
What are the most common TrimRx side effects?
Nausea, diarrhea, constipation, vomiting and stomach pain. TrimRx also lists burping or reflux, tiredness, headache, dizziness and soreness where you inject. On brand-name Wegovy and Zepbound, nausea was the most common, at 25% to 44%.
How long do TrimRx side effects last?
TrimRx doesn't give a time frame. Side effects are most likely when you start and after each dose increase. On Zepbound, most people who stopped because of side effects did so in the first few months. If side effects aren't getting better, message your Care Team.
Does TrimRx cause hair loss?
TrimRx doesn't list hair loss. On the brand-name labels, hair loss was reported in 3% (Wegovy) and 4% to 5% (Zepbound) of people, vs. 1% on placebo. On Zepbound, it was more common in women and was linked to weight loss.
Are TrimRx tirzepatide side effects worse than semaglutide?
In the brand-name labels, stomach side effects were reported more often with Wegovy than Zepbound. Those were separate trials, and TrimRx's compounded versions haven't been tested. Ask your provider which fits you.
Can TrimRx cause low blood sugar?
TrimRx lists low blood sugar for people with type 2 diabetes. The FDA labels say the risk goes up if you also take insulin or certain diabetes pills. Tell your provider about every medicine you take.
When should I go to the ER on TrimRx?
TrimRx says to call 911 for trouble breathing, swelling of your face, lips, tongue or throat, severe chest pain, or any emergency. Severe stomach pain that won't go away also needs fast care, since it can be a sign of pancreatitis.
