Collective Tirzepatide Reviews: What Members Report

By WPG Research Team · Last verified · Last updated

Reviews of Collective's compounded tirzepatide are strongly positive so far. On Trustpilot, Collective (collective.org) scores 4.8 from 282 reviews. Reviewers praise the $69 price at every dose, fast cold shipping, and an included anti-nausea drug. The main complaint is slow support replies. Collective is new, the drug is compounded and not FDA-approved, and there's a $199 yearly membership.

Below: where these reviews come from, what reviewers like and dislike, what tirzepatide really costs at Collective, and what we could not verify.

Weight Loss Provider Guide is an independent comparison resource for GLP-1 telehealth providers.

Bottom line: Collective is one of the lowest-priced compounded tirzepatide options we've checked, and early reviews are good. It's a new company, so the review record is short.

Best for: Cash payers who want compounded tirzepatide, especially at higher doses, and plan to stay several months.

Not best for: People who want an FDA-approved drug or insurance coverage, or who live in AL, CA, LA, MA, MS or WA. Anyone pregnant or under 18. GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.

What matters most: Use the 21-day trial. Try one vial, and cancel before day 21 if it's not for you.

Read these with care:

  • The record is short. Collective's Trustpilot page was claimed in July 2026.
  • Collective asks customers for reviews. Trustpilot says Collective invites customers to review and pays for a Trustpilot subscription. Invited reviews tend to skew positive.
  • Some reviews come before the medicine does. At least one tirzepatide review was written before the reviewer got their first order.
  • Watch the name. Trustpilot also lists collective.com, a different company. This page covers collective.org only.

Collective's tirzepatide page shows a 4.7 Trustpilot badge. The live Trustpilot page showed 4.8 on November 2.

What reviewers like about Collective tirzepatide

These themes came up again and again in the tirzepatide reviews we read.

1. The same price at every dose. This is the top theme. Reviewers on higher doses, including 10 mg and 15 mg, say they pay the same $69. Several did the math themselves: $69 a month plus $199 a year comes to about $85 a month. Many said they pay far less than at their old provider.

2. Easy switching. Many reviewers came from another compounded tirzepatide provider. Several say Collective's doctor kept them on their current dose.

3. Fast approval and delivery. Some reviewers report approval the same day or within a day. Most who mention timing say their vial arrived within about two to five days.

4. Cold, careful packing. Reviewers describe a large insulated box with several ice packs, still cold on arrival.

5. Anti-nausea medicine included. Several reviewers were surprised to get an anti-nausea drug (ondansetron, also called Zofran) with their order, even if they didn't need it.

6. Dietitian support. Reviewers mention live webinars and group calls with registered dietitians, plus messaging their doctor through the online portal.

7. The free trial. Reviewers who were unsure about the $199 fee liked getting a first vial during the 21-day trial before paying it.

Trustpilot's AI summary of recent reviews matches this. It cites affordable pricing, easy sign-up, a supportive medical team, educational webinars and timely shipping.

What reviewers complain about

The complaints are fewer, but they're consistent.

1. Slow support replies. This is the main one. Trustpilot's AI summary says some reviewers found support slow on dose questions or missing items, and felt left waiting at busy times.

2. Silence after approval. The one 3-star tirzepatide review describes several days with no update after approval and slower replies than the 24 hours support promised. That reviewer also said there was no phone number to call. Collective replied publicly, and the reviewer later updated that the delay was explained.

3. Shipping time varies. On Reddit, one commenter said shipping took about two weeks. Others in the same thread reported same-day shipping or fast 2-day air.

4. Monthly shipments only. Collective ships one month at a time. One Reddit commenter liked that it avoids prepaying. Another worried that one-month shipments raise the risk of refill delays.

5. The yearly fee and the fine print. Reddit commenters advised trying the first vial and canceling before the trial ends if you're unsure. One pointed out that Collective's terms don't lock in the $69 price. That's true: the terms say "Prices are subject to change at any point in our sole discretion."

6. A young company. Collective says on Trustpilot that it is "still a young company." One reviewer noted you can't choose your pharmacy and the company has a short track record.

Glycine and the pharmacy: what we could and couldn't confirm

Glycine. At least one Trustpilot reviewer and Reddit commenters say their Collective tirzepatide was compounded with glycine, an amino acid some pharmacies add. Collective's own pages do not list the formulation or any added ingredients. We could not confirm whether every Collective vial contains glycine. Ask your Collective doctor what's in your vial before you start.

The pharmacy. Collective says each prescription is filled by a "state-licensed US pharmacy" and "every batch is third-party tested for purity and potency." Its site does not name the pharmacy or the testing lab. Some reviewers name the pharmacy their vials came from. We have not confirmed that, so we don't repeat it here. You can ask Collective which pharmacy fills your order.

How much does Collective tirzepatide cost?

Collective says compounded tirzepatide is $69 a month at any dose. A $199 membership is billed once a year after a 21-day free trial.

How long you stayWhat you payTotal
1 month, cancel before day 21$69$69
6 months$199 + 6 x $69$613
12 months$199 + 12 x $69$1,027 (about $86/month)

What the membership covers, per Collective: a doctor-written plan, unlimited doctor messaging, check-ins and dose changes, nutrition and lifestyle support, live group Q&As with registered dietitians, and express cold shipping. The medicine is billed separately.

Fine print from Collective's terms:

  • The trial turns into a paid yearly membership unless you cancel. It renews every year.
  • Membership fees are non-refundable. There are no partial-period refunds.
  • Prices can change. Membership price changes are announced in advance and start at your next membership period.
  • Collective may charge a "nominal, uniform technology transaction fee" for each e-prescription.

Refund if you're not approved: Collective's FAQ says if the doctor decides it isn't right for you, "you get a full refund," and you're never charged for medicine you don't receive.

Insurance and HSA/FSA: Collective doesn't bill insurance. It says your medicine and membership "may qualify for HSA/FSA reimbursement." Your plan decides.

One reviewer said Collective waived their $199 fee through a financial aid program. Collective also says it gives a membership to someone in need for every paid one. We could not confirm how either works.

Is compounded tirzepatide FDA-approved?

No. Compounded tirzepatide is made by a pharmacy, not by Eli Lilly. Compounded drugs are not FDA-approved. The FDA says it does not review them for safety, effectiveness or quality before they're sold.

The FDA-approved brands are Zepbound and Mounjaro. They are not what Collective sells.

Collective says that in clinical studies, people on tirzepatide lost "an average of about 20–22%" of their body weight with diet and lifestyle changes. Those studies tested the brand-name drug, not compounded versions. Your results may differ.

If you want an FDA-approved drug or insurance help, see GLP-1 providers that accept insurance.

Side effects: what reviewers and Collective say

Tirzepatide's most common side effects are in the gut: nausea, vomiting, diarrhea and constipation. They're most common when you start and after a dose goes up.

Collective says its doctors may:

  • Offer an optional twice-a-week dosing plan to ease the nausea some people get with once-a-week shots.
  • Prescribe an anti-nausea medicine "when it helps."
  • Offer lower "microdose" options if big weight loss isn't your goal.

Reviewers back up the anti-nausea part. Several got ondansetron with their first order. Some switchers said it worked as well as what they used before. That's their experience, not a test.

Switching to Collective tirzepatide

Many reviewers switched from another provider. Collective says:

  • You can keep your current dose if its doctor approves.
  • Start about a week before your next dose is due, so the new vial arrives in time.

Cancel your old provider on its own schedule so you aren't billed twice.

Is Collective legit?

Here's what we found:

  • Who runs it: Health for Everyone Inc., doing business as CollectiveOS, in Houston, Texas. Collective says its CEO, Gunnar Lovelace, co-founded Thrive Market.
  • Doctors and pharmacies: Collective's terms say they are independent third parties.
  • FDA: We found no FDA warning letter naming Collective, CollectiveOS or Health for Everyone Inc. as of November 2, 2026. That does not mean the FDA has reviewed its drugs.
  • Disputes: Collective's terms send disputes to binding arbitration.
  • Track record: Short. Its Trustpilot profile was claimed in July 2026.

The reviews so far point to a real company shipping real medicine. The open questions are the unnamed pharmacy, the formulation, and how support holds up as it grows.

Who is Collective tirzepatide good for?

A good fit if you:

  • Want compounded tirzepatide at a flat, low price.
  • Are on, or moving toward, a higher dose. The price doesn't change.
  • Plan to stay several months, so the $199 spreads out.
  • Don't want to prepay months of medicine.

Not a good fit if you:

  • Want an FDA-approved drug or insurance coverage.
  • Live in Alabama, California, Louisiana, Massachusetts, Mississippi or Washington.
  • Need a phone line or same-day support replies.
  • Want to know your pharmacy and formulation before you sign up.

Who should talk to a doctor first?

See your own doctor before using any GLP-1 if you:

  • Are pregnant, trying to get pregnant, or breastfeeding.
  • Are under 18.
  • Have had pancreatitis.
  • Take insulin or other diabetes drugs.

GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.

A licensed clinician decides if tirzepatide is right for you. Collective does not promise a prescription.
