Here's the catch: $299 is where the standard price starts. It isn't always where it stays. Below you'll find every dose price from Willow's own list, what 12 payments add up to, the $150 fee that only hits one kind of patient, and the cancel rule that Willow's own pages don't agree on.

"Compounded" means a pharmacy mixes the medicine for you. Compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality.

How much does Willow GLP-1 cost per month? (Full price list)

Willow’s regular GLP-1 prices start at $299 a month for compounded semaglutide — shots up to the 3.5 mg tier, or tablets at any strength. Compounded tirzepatide shots cost $399, $499, $524 or $549 a month depending on strength, and tirzepatide or combination tablets cost $399 or $459. These prices come from Willow's own help-center price list, checked September 23, 2026.

Why a headline price isn't the full price list

Willow's product pages lead with starting prices. Its separate Help Center page lists the higher-price tiers too. We compared both so you don't have to mistake a starting price for a promise about every refill. Product page · Full price list

Your starting price vs. your real price

When you sign up, you need three numbers, not one:

1. What you pay today.
2. What your next refill costs.
3. How often you're billed.

Willow’s Terms of Service say the total and the billing interval are shown at checkout. Write all three down before you pay.

Why the help page says “plans start at $199”

Willow's price page opens with "plans start at $199." That $199 is NAD+, a separate wellness shot. It is not a weight-loss GLP-1. The regular GLP-1 starting price on that list is $299. Separately, a qualifying $100 friend-referral discount can reduce a $299 first payment to $199. See the referral terms below.

Did Willow lower its tablet price?

Use $299 for the current tablet budget—not an unverified savings claim. The current list tells us what Willow publishes now. It does not prove when a price changed or that every other price stayed the same.

Don't compare Willow's milligrams to Wegovy or Zepbound

Willow's strengths (like 2.125 mg or 13.5 mg) don't line up with brand-name doses. That's because compounded medicines are made by Willow's partner pharmacies and are not the same product. Why is Willow GLP-1 so expensive?

Willow isn’t the cheapest compounded GLP-1 program; other providers publish lower starting prices than its $299. Willow’s standard price bundles the doctor, the medicine and shipping into one monthly charge, with no separate membership fee on top. Whether that’s worth it depends on your medical needs, your full quote and how much you value monthly billing.

For example, Mochi publishes compounded semaglutide from $60 plus its ongoing $79 membership: $139 before any separately charged services. What Willow’s standard monthly plan gives you is a bundled bill without an annual prepayment. Check the total and renewal interval at checkout. Even some happy patients admit it costs more. One four-month patient wrote that Willow is

"a bit more expensive than other sites for GLP, I am sticking with them"

The honest picture from reviews: On Trustpilot, Willow scores 3.9 out of 5 from 356 reviews (September 23, 2026). About 58% are five stars and about 31% are one star. Will my Willow price go up at a higher dose?

Yes, for some medicines. Willow's tirzepatide shot rises from $399 to $549 a month as the strength goes up, the semaglutide shot rises from $299 to $399 at the 7 mg tier, and tirzepatide tablets go from $399 to $459. Semaglutide tablets stay $299 at every strength, and Willow's Terms say it will tell you a new price before charging it.

Where the price steps happen

Willow’s Terms of Service say your monthly fee can go up or down if your prescription changes, like a dose increase. They also say Willow will tell you the new amount, and get any consent the law requires, before it charges you.

What 3, 6 and 12 payments cost

If your price stays the same for each payment, here's the math:

A price change adds up fast. Say you make three payments of $399, then nine of $499. Your 12 payments total $5,688. That’s $900 more than twelve payments at $399.

What does Willow's price include — and what costs extra?

Willow says its standard monthly price includes the online visit, the prescribed medicine, unlimited messages with your Willow doctor, 24/7 care-team support and free shipping. Using your own pharmacy changes the bill: Willow charges a $150-a-month platform fee in that case, and you pay the other pharmacy for the medicine separately.

The $150 fee for using your own pharmacy

Willow lets you send your prescription to a pharmacy you choose. But it doesn't recommend it. Willow says most pharmacies can't make custom compounded medicines, and pharmacies can refuse the prescription.

If you go that route anyway, Willow charges a $150 monthly platform fee. Then you pay the outside pharmacy for the medicine on top of that.

Here's the math that matters: for an outside pharmacy to beat Willow's standard $299 semaglutide price, it would have to sell you the medicine for less than $149 a month. At exactly $149, the two routes tie before any other fees. Above $149, the outside route costs more; below it, it costs less. Shipping is free. Speed is a different question.

"Free 2-day shipping" starts after the pharmacy makes your order. Willow says pharmacy processing takes 2–4 business days, and delivery then takes 2–3 days. That is pharmacy processing plus transit—not a promise of arrival two days after approval. Weekends, holidays and fulfillment delays can add time. Willow’s shipping explanation

If a package gets lost or sits in heat too long, Willow says it will arrange a replacement. Contact the care team right away with your tracking number. Ask the dispensing pharmacist whether a delayed or temperature-exposed medicine is usable; do not assume a replacement policy answers that safety question. Willow FAQ

Is Willow cheaper than FDA-approved Wegovy or Zepbound?

Sometimes—not across every prescription or payment plan. Willow’s $299 semaglutide shot tier is $50 below Wegovy’s regular $349 medication-only offer, but some Willow shots cost $399 and Wegovy has a lower introductory offer. For tirzepatide and pills, check the FDA-approved option’s price and the prescriber fee together before deciding.

Here’s where a lower drug price can disappear once you add the care fee.

Important: Willow's medicines are compounded. The FDA does not review compounded drugs for safety, effectiveness or quality. They are not the same products as Wegovy® or Zepbound®, and they are not generics. The tables below compare prices, not medicines or doses.

To get a brand-name drug, you need someone to prescribe it. So we priced three ways to do that: your own doctor, Ro's annual plan and Ro's monthly plan. Ro advertises the same base cash-pay medicine prices as LillyDirect and NovoCare, plus a membership: $39 for the first month, then as low as $74/month with an annual plan paid upfront, or $149/month month to month. Read the annual row carefully: $74 is a monthly equivalent, not a monthly bill. The annual membership is paid upfront. The tables pair one medication fill with one month of care so you can compare the price components; they do not show your actual invoice schedule.

A branded injection fill here generally supplies 28 days, while the Wegovy pill prices cover 30 tablets. Prescriber visits, membership months and medication fills may follow different schedules. Semaglutide shot: one fill plus care

Wegovy pen prices from NovoCare: eligible new patients can pay $199 for each of the first two 28-day fills of the 0.25 mg and 0.5 mg starter doses through December 31, 2026. The regular offer is $349 per fill for doses up to 2.4 mg. Wegovy HD 7.2 mg is $399 and is not included in this table. Offer eligibility and refill dates matter.

A 12-payment comparison—not a calendar-year quote:

• Willow: Twelve payments at $299 total $3,588. Confirm how many days each payment covers.
• Wegovy + Ro annual plan: Twelve medicine fills at $199 twice and $349 ten times total $3,888. Add the initial $39 care fee and the full annual membership charge shown at checkout. Using 12 × $74 = $888 as an illustration of that prepayment gives $4,815 paid. The $888 is calculated from the advertised monthly equivalent, not a tested checkout quote; confirm the actual upfront amount. This model buys the first care month plus a further 12 months, so it also prepays care beyond the first year.
• Wegovy + Ro monthly plan: The same twelve medicine fills plus $39 + 11 × $149 in care fees total $5,566—$1,978 more than twelve $299 Willow payments, before differences in days supplied or extra charges.
• Wegovy + your own doctor: Those twelve medicine fills total $3,888, plus applicable care costs—$300 more than twelve $299 Willow payments before those costs.

Do not count an annual plan as eleven small monthly payments after the introductory month. You must fund the full annual prepayment. All examples assume the stated prices remain available; they are not price guarantees for 2027.

Our read: Willow’s $299 injection tier is cheaper than the standard, undiscounted Ro routes shown after the Wegovy starter offer. That is a narrow price finding—not proof that Willow is cheaper at every dose, that all routes cover the same number of days, or that a compounded medicine is the right choice for you.

Tirzepatide shot: one fill plus care

Current Lilly KwikPen self-pay terms list $299 at 2.5 mg, $399 at 5 mg, and $449 at 7.5–15 mg with the applicable offer. The $449 offer applies to the first eligible higher-dose purchase; continuing that offer requires refilling within 45 days of receiving the previous prescription. A fill supplies 28 days. The 2.5 mg dose is a starting dose, not a maintenance dose.

Miss the offer’s timing or eligibility conditions and the higher-dose bill can be different. Lilly lists regular KwikPen prices of $499 at 7.5 mg and $699 at 10–15 mg. The discounted prices are not unconditional price caps.

Our read: The lowest and highest published price points are close once Ro’s care fee is included: the annual-plan equivalents are $26 below Willow’s range endpoints, while the monthly-plan figures are $49 above. Without a separate telehealth membership, the discounted Zepbound endpoints are $100 below Willow’s endpoints, before your own doctor’s costs. Those are comparisons between price ranges, not matched doses or equal treatments.

Pills: one fill plus care

Wegovy pill prices from NovoCare: $149 at 1.5 mg, $199 at 4 mg, and $299 at 9 mg and 25 mg, for a bottle of 30 tablets. These are medication prices, not care-inclusive totals.

Willow’s tablet dissolves under the tongue. The FDA-approved Wegovy pill is swallowed whole. They are different formulations; this price table does not establish equal absorption, results or safety. Willow FAQ · Wegovy instructions

Our read: Compare the prescription and care fee together. At the lowest listed Wegovy pill price, the modeled Ro totals are below Willow’s $299; at $299 for the brand medication, adding a Ro membership makes the total higher. The middle tier depends on the care plan. We did not establish whether tablet complaints are more common than injection complaints, and online reviews cannot prove either formulation’s effectiveness.

One difference that can change everything: who qualifies

Willow's published eligibility list doesn't give a BMI cutoff. That does not mean it has no medical criteria—or that it is a workaround when a branded medicine is unsuitable. For weight management, branded treatments have their own obesity or overweight-with-related-condition criteria, and a prescriber must confirm whether a specific drug fits you. Does insurance cover Willow GLP-1? (Plus HSA, FSA and discounts)

Willow is a self-pay service: it doesn’t bill insurance, and its August 14, 2026 Terms say patients agree not to submit claims to insurers. An older help article discusses reimbursement, so the documents conflict. Willow accepts HSA and FSA cards and offers receipts, but tax savings depend on qualified expenses and your own benefits—not just whether a card goes through.

Insurance: don't count on it

Willow's own documents disagree here:

The newer document is the Terms you agree to. And even the older help article says Willow can’t give insurers additional requested paperwork beyond its HSA/FSA receipt. HSA and FSA: tax savings, not a lower Willow bill

An HSA (health savings account) or FSA (flexible spending account) holds money you set aside before taxes for medical costs. Willow accepts both cards at checkout.

Your card still pays the full bill. The potential saving comes from taxes you do not pay on qualifying contributions. Here is one example using a qualifying employer payroll arrangement, with the whole expense eligible and the full 7.65% employee payroll-tax saving applying:

IRS rules generally treat prescription medicine and weight-loss treatment for a doctor-diagnosed disease, such as obesity, as qualified medical expenses. How to get your Willow HSA receipt: Log in to your Willow patient portal → click the menu under your name (top right) → choose Insurance → click Download next to HSA Receipt → pick the year.

Heads up: Your benefit administrator may ask for a letter of medical necessity (a note from a doctor explaining the medical need). Willow’s help article limits additional documents requested by insurance companies; it does not clearly settle whether your clinician can supply the letter your HSA/FSA administrator needs. Promo codes and first-month deals

Willow publishes a $100 friend-referral offer. Its Help Center says an existing patient can share a referral link, and a new patient using that link should see a $100-off banner during onboarding and the discount at checkout. The referrer also receives a $100 credit for a successful referral. Willow’s referral program

Our math: A confirmed $100 discount would reduce a $299 first bill to $199, or a $399 first bill to $299. It does not reduce every future payment.

Money tight? Ask about pausing

One patient wrote that she had to pause her account for six months during a tough financial stretch, and "it was quick and easy to do so." (Sara, Trustpilot review, September 3, 2026)

We did not verify a formal pause policy in the documents reviewed. Ask the care team how a pause works, whether it stops your next charge and what happens to an order already processing.

What not to do

Don’t change your dose or stretch your medicine on your own to save money. Your dose is a medical decision. Talk to your Willow doctor about your budget instead.

Can I cancel Willow before my next charge?

Yes, but timing matters. Willow’s own pages give different deadlines—two business days or two calendar days before processing, or two calendar days before shipping. Ask Willow to confirm the exact cutoff in writing, and act early. Its cancellation policy says an order cannot be changed or canceled once submitted to the pharmacy.

1. Find your next processing date in your Willow portal, then ask the care team for the exact cancellation cutoff, time and time zone.
2. Use the account’s cancellation option if available and send a message to the care team. Aim for at least 3 business days before processing as an early reminder, not a guaranteed deadline; allow more time for holidays or unclear dates.
3. Save screenshots and get written confirmation that future renewals are stopped.

The three-business-day target is our planning buffer, not a deadline confirmed by Willow. A changed processing date, holiday or order already submitted can still change what is possible.

Willow’s Terms say that if a doctor doesn’t prescribe, Willow declines the order and returns your payment the same way you paid, even if the order was already accepted.

Willow serves 36 states. It does not serve Alabama, Alaska, Arkansas, Delaware, Kansas, Louisiana, Mississippi, New Mexico, North Dakota, Rhode Island, South Dakota, Utah, Vermont, West Virginia or Washington, D.C. Willow also can't accept people with diabetes, or anyone who is pregnant or breastfeeding.

Willow also screens out people with an eating disorder, recent bariatric surgery, gallbladder disease, pancreatitis, or a history of medullary thyroid cancer or MEN2. A Willow doctor makes the final call.

Willow can be worth considering if its full monthly quote works for you and your clinician identifies a need for a compounded prescription. The $299 price is not flat across every semaglutide injection tier. For tirzepatide or tablets, compare the relevant FDA-approved route too. What Willow no longer claims

In December 2025, the National Advertising Division (NAD — an industry group that reviews ad claims) said Willow hadn't backed up certain ad claims for its compounded semaglutide and should stop or change them. Willow first disagreed, and NAD referred the matter to regulators. In July 2026, after talks with the FTC, Willow told NAD it had permanently dropped the challenged claims. What that means for your wallet: Consider Willow only when the prescription is medically appropriate and the price, service and access fit you. Not because of results promises the company itself has withdrawn. Today, Willow’s own site states plainly that its compounded medicines are not FDA-approved and are not brand-equivalent.

Could Willow's price change later?

Yes. Willow’s Terms allow price changes, including changes tied to your prescription, with the stated notice and consent requirements. On April 30, 2026, the FDA proposed excluding semaglutide, tirzepatide and liraglutide from the 503B bulks list—the list relevant to certain bulk-ingredient compounding by outsourcing facilities. That proposal is not a blanket ruling about every compounded prescription, and it does not establish a price or supply change at Willow. What we did not verify: a paid checkout, a refund, the price of add-ons like anti-nausea medicine, a formal pause policy, a redeemed referral discount, the exact annual-plan charge on a patient’s Ro checkout, a July 9 Willow price snapshot, or which pharmacy fills any one person’s order.

How we did the math: Willow totals multiply the published price by the number of payments. Price-change examples add the payments at each rate. The own-pharmacy break-even subtracts the $150 platform fee from the standard $299 Willow price. Brand tables combine one medicine fill with one month of care; annual membership figures are monthly equivalents, while the prepayment example separately shows cash required. We don’t estimate how long anyone stays on treatment.

Willow’s standard GLP-1 prices start at $299, but your exact treatment, renewal terms and any verified discount set the bill. These answers separate the published price from the parts you still need to confirm.

How much does Willow GLP-1 cost per month?

Willow’s regular price starts at $299 a month for compounded semaglutide; the listed 7 mg injection tier is $399. Tirzepatide shots cost $399 to $549, and tirzepatide and combination tablets cost $399 or $459. Prices were checked on Willow’s own list September 23, 2026.

Is Willow GLP-1 $299 at every dose?

Does Willow have a membership fee?

There is no separate membership fee added to Willow’s standard medication-inclusive price. Using an outside pharmacy is a different arrangement: Willow publishes a $150 monthly platform fee, and you pay the pharmacy for medication separately. Fee policy

Does insurance cover Willow GLP-1?

Do I pay if Willow doesn't approve me?

Willow’s Terms say that if a doctor does not prescribe, Willow declines the order and returns your payment through the original payment method. That is its written policy, not a refund we tested. Payment terms

How do I cancel Willow without another charge?

Ask Willow for the exact processing date and cancellation cutoff, use its available cancellation method, message the care team and save confirmation. Is Willow cheaper than Zepbound or Wegovy?

It depends on your prescription, the current offer and care fees. Willow’s $299 shot tier is below Wegovy’s regular $349 drug-only offer, but Wegovy’s introductory offer is lower and some Willow shots are $399. For tirzepatide and tablets, use the fill-plus-care tables above; a lower drug price alone does not settle the full bill.

Can I use my own pharmacy to save money?
