Quick fork — do you already have a Wegovy prescription?
- Yes → NovoCare Pharmacy is your cheapest path. Jump to pen-vs-pill pricing by dose.
- No → A full-service telehealth path is almost certainly your best move; which one depends on whether you want to try insurance first. Keep reading.

The right answer depends on whether you already have a prescription, whether you have commercial insurance, and whether you want to fight for coverage before going cash-pay. Wegovy is prescription-only.
The Three Prices That Actually Matter for Wegovy Cost Without Insurance
| Price type | What it is | Who actually pays it |
|---|---|---|
| List price ($1,349.02/package) | Novo Nordisk's official sticker price before any discount | Patients whose insurance processes Wegovy at full retail with no coverage support |
| Official self-pay ($149–$399/month) | Novo Nordisk's direct cash-pay pricing through NovoCare Pharmacy | Uninsured patients and commercially insured patients whose plan doesn't cover Wegovy — excluding government program beneficiaries |
| Telehealth all-in | NovoCare-matched medication pricing + the telehealth provider's monthly membership fee | People who need a prescription AND ongoing clinical support |
The Complete 2026 Wegovy Self-Pay Price Matrix (Verified April 21, 2026)
Wegovy Self-Pay Reality Matrix — Every Route Compared
| Route | First-month total (std pen) |
|---|---|
| ✅ NovoCare Pharmacy (direct) | $349 |
| NovoCare at retail (CVS, Walgreens, Costco, 70k+ pharmacies) | $349 |
| Hers / Hims (Novo partner, March 2026) | Confirm at checkout |
| Ro — monthly cash-pay | $238 first / $498 ongoing (monthly) or $423 (annual) |
| Ro — 12-month subscription ⭐ | $323 first month (sub + $74 Ro Body annual) |
| WeightWatchers Med+ (Novo partner) | Varies by commitment |
| LifeMD (Novo partner) | ~$274 on current promo |
| Costco Pharmacy via Sesame | $349 med + Sesame subscription |
| GoodRx (Novo partner program) | Matches NovoCare when eligible |
| TrumpRx (federal cash-pay platform) | ~$350/mo (verify current terms) |
| ❌ Novo Nordisk PAP | Not covered — see note below |
• *Wegovy pill 4 mg is $149/month through August 31, 2026, then $199/month after.
• Pen intro ($199/month) for new-patient 0.25 mg or 0.5 mg doses runs through June 30, 2026.
• Government beneficiaries (Medicare, Medicaid, TRICARE, VA) are NOT eligible for NovoCare self-pay pricing — even electing to self-pay outside coverage.
• Novo Nordisk’s PAP does not cover Wegovy for weight loss in 2026. The PAP covers certain diabetes medications (like Ozempic) but not Wegovy. We verified this against current NovoCare program materials.
• All medication prices are Novo Nordisk’s manufacturer-set cash-pay pricing — the drug cost is the same across every authorized partner. Differences show up in the membership fee layered on top.

Compare providers →Wegovy Pill vs Pen Cost Without Insurance

The pill requires a daily empty-stomach routine; the pen is once weekly with no fasting required. Both are FDA-approved Wegovy options from Novo Nordisk.
| Factor | Wegovy Pen | Wegovy Pill |
|---|---|---|
| Dosing schedule | Once weekly | Once daily |
| Starter dose cash price | $199/mo (intro, 0.25–0.5 mg through Jun 30, 2026) or $349/mo standard | $149/mo (1.5 mg); $149/mo for 4 mg through Aug 31, 2026 |
| Maintenance dose cash price | $349/mo (1 mg / 1.7 mg / 2.4 mg) | $299/mo (9 mg or 25 mg) |
| High-dose option | Wegovy HD 7.2 mg — $399/mo | Not available in HD |
| Avg weight loss (max adult dose, clinical trial data) | ~15% on 2.4 mg; ~19% average on 7.2 mg HD | ~14% on 25 mg |
| Storage | Refrigerated | Room temperature |
| Daily-routine burden | Inject once weekly, any time | Empty stomach, ≤4 oz plain water, wait 30 min before food/drink/other oral meds |
| Intro offer expiry to watch | June 30, 2026 ($199/mo pen intro) | August 31, 2026 ($149/mo for 4 mg) |
| Which is cheaper self-pay? | $349–$399/mo | $149–$299/mo ✅ |
When the pill makes sense for cost-driven buyers
- Your clinician agrees the pill is clinically appropriate
- You can reliably take it on an empty stomach before your first food of the day
- You prefer a pill over weekly injections
- You want the lowest self-pay cost for brand-name semaglutide
When the pen is worth the extra $50–$100/month
- You don't want to reorganize your morning around a 30-minute fasting window
- You want the higher-dose option (Wegovy HD 7.2 mg)
- Your insurance is more likely to cover the pen (true for most commercial plans)

The $25 Wegovy Savings Offer: Who It Works For (And Who It Doesn’t)
The four real scenarios
| Your situation | Your likely cheapest path |
|---|---|
| Commercial insurance that covers Wegovy | Use the Wegovy Savings Offer ✅ |
| Commercial insurance that excludes Wegovy | NovoCare self-pay ($149–$399/mo) is usually cheaper |
| Uninsured / self-pay | NovoCare direct at $149–$399/month (pen standard or pill by dose) |
| Medicare, Medicaid, TRICARE, VA, or any government program | See government insurance section below for Medicare GLP-1 Bridge and BALANCE Model |
Don’t guess about your formulary. A free insurance check tells you in minutes whether your plan covers Wegovy and whether a prior authorization is required.
Check your GLP-1 coverage free through Ro → Ro contacts your insurer and emails a personalized reportThe Cheapest Legitimate Way to Get Wegovy Without Insurance
Case 1: You have commercial insurance that covers Wegovy
Use the Wegovy Savings Offer.
Enroll at novocare.com or text SAVE to 83757 and take the offer to your pharmacist. With commercial coverage, you'll typically pay $0–$25/month with a maximum savings of $100/month. This is the cheapest legitimate path for anyone whose plan already includes Wegovy.
Case 2: You already have a prescription but no coverage (or a plan that excludes Wegovy)
Go straight to NovoCare Pharmacy.
No middleman, no membership fee. Two ways: (1) NovoCare Pharmacy home delivery — enroll at novocare.com, prescription ships directly. (2) NovoCare coupon at retail — take the offer to any of 70,000+ pharmacies (CVS, Walgreens, Walmart, Costco) and fill locally at the same price. You pay $149/mo (pill 1.5 mg), $149/mo (pill 4 mg through Aug 31, 2026), $299/mo (pill 9 mg or 25 mg), $199/mo (pen intro through Jun 30, 2026), $349/mo (pen standard), or $399/mo (Wegovy HD 7.2 mg).
Case 3: You still need a doctor AND medication
Compare the real first-month total, not just the medication price.
All three of the strongest options carry the same Novo-partner medication pricing. The differentiator is the experience: Hers — best for women who want a mainstream consumer-health brand. Hims — same offering, male-coded. Ro — best if you want someone to fight insurance first (free coverage checker, insurance concierge, prior auth support as part of membership). For the lowest locked medication price with a 12-month commitment, the Novo multi-month subscription through Ro drops the pen to $249/month flat.
Case 4: Low income and uninsured (below ~200% federal poverty level)
Novo PAP does NOT cover Wegovy for weight loss.
As of 2026, the Novo PAP covers certain diabetes medications (including Ozempic) but does NOT cover Wegovy for weight loss. We verified this directly against the current PAP covered-products list. Realistic cheapest paths for low-income uninsured patients: (1) NovoCare Pharmacy self-pay at $149/month for the pill starter — still the lowest clean brand-name price. (2) Medicare GLP-1 Bridge at $50/month launching July 1, 2026 for eligible Part D enrollees. (3) State Medicaid in one of the 13 states whose Medicaid programs currently cover GLP-1s for obesity treatment.
Hers vs Hims vs Ro vs Sesame vs LifeMD: Which Telehealth Path Fits You?
| Feature | Hers | Hims | Ro |
|---|---|---|---|
| FDA-approved Wegovy pen | ✅ | ✅ | ✅ |
| FDA-approved Wegovy pill | ✅ | ✅ | ✅ |
| Medication price baseline | NovoCare match | NovoCare match | NovoCare match |
| 12-month sub available | Rolling out | Rolling out | ✅ Live ($249/mo pen) |
| Membership/program fee | Required; check current | Required; check current | $39 first mo, then $149/mo (or $74/mo annual prepay) |

| Costco pickup | — | — | — |
| Best for | Women, consumer-brand app-first | Men, same structure as Hers | Unisex, insurance-fighting, coverage uncertain |
Telehealth doesn’t beat NovoCare on medication price. It beats NovoCare on everything around the medication — evaluation, prescribing, insurance fighting, and follow-up care. If what you actually need is the whole pathway, that’s what the membership fee is buying.
Who should pick each one
Pick Hers if…
You're a woman who wants a mainstream consumer-health brand with FDA-approved Wegovy and a clean app-first flow.
Check your Hers Wegovy options →Pick Hims if…
Same logic, male-coded. Same medication access through the March 2026 Novo partnership, same underlying pricing structure.
Check your Hims Wegovy options →Pick Ro if…
You're not 100% sure insurance won't cover Wegovy. Ro's free GLP-1 coverage checker contacts your insurer directly and emails a personalized report. Insurance concierge included in membership.
Run Ro's free GLP-1 coverage check →Pick Sesame (Costco path) if…
You're a Costco member. Success by Sesame gives 10% off the subscription, and once you have a prescription you can pick up Wegovy at Costco Pharmacy at NovoCare-matched pricing. Costco Executive members and Costco Anywhere Visa holders also receive 2% back on the medication.
See the Costco Pharmacy path through Sesame →What to Do If Coverage Was Denied, Your Deductible Is Too High, or You’re on Government Insurance
If your commercial insurance denied Wegovy
Don’t accept the first denial. Appeals are a structured process, and a well-documented request succeeds more often than readers expect:
- 1Ask your doctor to file a formulary exception citing medical necessity (BMI criteria, weight-related conditions, cardiovascular risk)
- 2If denied again, request an external independent review
- 3While the appeal is pending, use NovoCare self-pay or the Wegovy pill ($149/month low dose) as a bridge so your weight-loss timeline doesn't stall
We maintain a separate guide: Wegovy Prior Authorization: Criteria, Forms & What to Do If Denied.
If your plan excludes weight-loss drugs entirely
Some employer plans specifically exclude weight-loss medications. Appealing won’t unlock coverage — the exclusion is the plan design. One exception: Wegovy is also FDA-approved to reduce the risk of major cardiovascular events in adults with established cardiovascular disease who are overweight or obese. Some plans that exclude weight-loss drugs will cover Wegovy under the cardiovascular indication. If you have qualifying cardiovascular history, ask your doctor whether that pathway applies.
If your deductible makes insurance useless
HSA/FSA note: Most providers including NovoCare accept HSA/FSA payment at checkout. Ro is an exception — Ro does not accept HSA/FSA cards directly. You pay with a regular card and submit receipts for reimbursement. Hers and Hims recommend the same reimbursement approach.
If you’re on Medicare
Medicare GLP-1 Bridge
CMS demonstration program running July 1, 2026 – December 31, 2026. Eligible Medicare Part D enrollees can access Wegovy and Zepbound for weight management at $50/month. Eligibility: BMI ≥ 35, or BMI ≥ 27 plus other clinical criteria.
BALANCE Model
Launches in Medicare Part D January 1, 2027 and in participating state Medicaid programs as early as May 2026. CMS negotiates GLP-1 pricing directly with manufacturers on behalf of participating plan sponsors.
If you’re on Medicaid
Medicaid coverage for Wegovy varies by state. As of January 2026, KFF identified 13 state Medicaid fee-for-service programs that cover GLP-1s for obesity treatment (usually with prior authorization). California’s Medi-Cal stopped covering Wegovy for weight management on January 1, 2026. State Medicaid rules change frequently — check your state’s Medicaid formulary directly. The BALANCE Model opens to state Medicaid agencies as early as May 2026.
If Brand-Name Wegovy Is Still Above Budget
Route 1: Zepbound via LillyDirect
Eli Lilly's FDA-approved tirzepatide self-pay vials: $299/month (2.5 mg), $399/month (5 mg), $449/month (7.5–15 mg). In head-to-head trials tirzepatide produced more weight loss than semaglutide. If clinical results at a lower cost than the Wegovy pen is your priority, Zepbound is worth discussing with your clinician.
See cheapest Zepbound optionsRoute 2: Foundayo (orforglipron) via Ro
Eli Lilly's FDA-approved oral GLP-1, available through Ro. If the pill format matters and you want a second oral option besides Wegovy pill, Foundayo is worth considering.
Route 3: Medicare GLP-1 Bridge
$50/month for eligible Medicare Part D enrollees, July 1–December 31, 2026. Covered above.

What the Public Review Landscape Shows
We’ve deliberately chosen not to publish individual user quotes. Reviews on third-party platforms are not independently verified, reflect single-user experiences, and for a YMYL medication decision we’d rather point you to the actual review platforms:
When reading public reviews: weigh recency (the telehealth Wegovy market changed significantly in March 2026), look for reviews about the specific GLP-1 service (not other services the provider offers), and note that testimonials on provider-owned pages are almost always compensated — Ro publicly discloses its member testimonials are compensated.
