2. BMI ≥30 with one of the following: heart failure with preserved ejection fraction (HFpEF), uncontrolled hypertension, or chronic kidney disease (CKD) stage 3a or higher, OR
3. BMI ≥27 with one of the following: prediabetes, previous myocardial infarction (heart attack), previous stroke, or symptomatic peripheral artery disease

You must also be enrolled in Medicare Part D or an MA-PD plan, and your prescriber must submit a prior authorization to the CMS central processor confirming the clinical criteria were met when GLP-1 therapy started.
