Hims Weight Loss Side Effects: What to Expect and When to Get Help
By WPG Research Team · Last verified · Last updated
Hims weight loss side effects are mostly gut problems: nausea, diarrhea, constipation, vomiting and stomach pain. They show up most when you start or raise a dose. On the brand-name labels for the GLP-1s Hims lists, nausea hit about 16% to 44% of people. Serious problems are uncommon but real.
Weight Loss Provider Guide is an independent comparison resource for GLP-1 telehealth providers.
The short answer
- Most side effects are stomach and gut related. Nausea is the one people talk about most. Diarrhea, constipation, vomiting and stomach pain come next.
- They're most likely early and after dose increases. Hims says some nausea in the first few weeks is normal, and that many side effects fade as your body adjusts.
- Hims doesn't publish its own rates. The best numbers come from the FDA labels for Wegovy, Zepbound, Foundayo and Ozempic — the brand medicines Hims actually lists.
- Some signs need fast help. Severe stomach pain that won't quit, swelling of your face or throat, or not keeping fluids down are not "just side effects."
GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Still deciding whether a Hims GLP-1 — or any GLP-1 path — fits you? The right next step depends on your state, budget, insurance and which medicine you want.

On November 6, 2026, Hims listed these GLP-1 options on its weight loss page. Side effects track the medicine, not the Hims app.
| Hims listing (Nov 6, 2026) | Active ingredient | FDA-approved for weight loss? |
|---|---|---|
| Wegovy pill (and microdosing plan) | Semaglutide (oral) | Yes |
| Wegovy pen | Semaglutide (weekly shot) | Yes |
| Foundayo pill | Orforglipron (daily pill) | Yes |
| Zepbound vial and KwikPen | Tirzepatide (weekly shot) | Yes |
| Ozempic pill and Ozempic pen | Semaglutide | No — approved for type 2 diabetes |
| Mounjaro | Tirzepatide | No — approved for type 2 diabetes |
Hims also lists a higher-priced Zepbound option at $1,899 a month alongside the $299 vial and KwikPen. A licensed Hims provider decides which medicine, if any, fits you.

How common are Hims weight loss side effects?
Hims does not publish how often its members get side effects. The closest real numbers are from the FDA labels for the brand-name drugs Hims sells.
Wegovy (semaglutide) — pill and pen
Wegovy injection Table 3 rates in adults with obesity or overweight (2.4 mg once weekly vs placebo). The Wegovy tablet label says common side effects in the 25 mg pill trial were similar in type and frequency to that same table.
| Side effect | Wegovy 2.4 mg injection | Placebo |
|---|---|---|
| Nausea | 44% | 16% |
| Diarrhea | 30% | 16% |
| Vomiting | 24% | 6% |
| Constipation | 24% | 11% |
| Stomach pain | 20% | 10% |
| Headache | 14% | 10% |
| Tiredness | 11% | 5% |
| Indigestion | 9% | 3% |
| Dizziness | 8% | 4% |
| Burping | 7% | Less than 1% |
| Hair loss | 3% | 1% |
In those injection trials, 6.8% of people on Wegovy 2.4 mg stopped because of side effects, vs 3.2% on placebo. In the Wegovy 25 mg tablet trial, 6.9% stopped vs 5.9% on placebo; gut problems were the top reason.
Zepbound (tirzepatide) — vial and KwikPen
Zepbound Table 1 rates (5 mg, 10 mg and 15 mg vs placebo) in adults with obesity or overweight:
| Side effect | Zepbound 5–15 mg | Placebo |
|---|---|---|
| Nausea | 25% to 29% | 8% |
| Diarrhea | 19% to 23% | 8% |
| Vomiting | 8% to 13% | 2% |
| Constipation | 11% to 17% | 5% |
| Stomach pain | 9% to 10% | 5% |
| Indigestion | 9% to 10% | 4% |
| Injection-site reactions | 6% to 8% | 2% |
| Tiredness | 5% to 7% | 3% |
| Burping | 4% to 5% | 1% |
| Hair loss | 4% to 5% | 1% |
4.8% to 6.7% of people on Zepbound stopped because of side effects, vs 3.4% on placebo. Most who stopped did so in the first few months, mainly for gut problems.
Foundayo (orforglipron) — daily pill
Foundayo Table 1 rates (5.5 mg, 9 mg and 17.2 mg once daily vs placebo):
| Side effect | Foundayo 5.5–17.2 mg | Placebo |
|---|---|---|
| Nausea | 26% to 35% | 10% |
| Constipation | 20% to 27% | 9% |
| Diarrhea | 21% to 25% | 11% |
| Vomiting | 13% to 24% | 4% |
| Indigestion | 12% to 16% | 4% |
| Stomach pain | 13% to 14% | 7% |
| Headache | 8% to 9% | 7% |
| Tiredness | 6% to 9% | 4% |
| Burping | 6% to 8% | 1% |
| Hair loss | 4% to 5% | 2% |
8% of people on Foundayo stopped because of side effects, vs 3% on placebo. Most of those stops were for gut problems. Nausea, vomiting and diarrhea were highest during dose increases and eased over time.
Ozempic (semaglutide) — if prescribed
Ozempic is FDA-approved for type 2 diabetes, not weight loss. In its diabetes trials (0.5 mg and 1 mg vs placebo), the most common side effects at 5% or more were nausea (about 16% to 20%), vomiting, diarrhea, stomach pain and constipation. Hims may prescribe it only when a provider decides you meet clinical eligibility.
What these tables tell you:
- Nausea is the big one across Wegovy, Zepbound and Foundayo.
- Many people stay on treatment. Stop rates for side effects were in the mid-single digits on most of these labels.
- The early months are the hardest. Dose increases are when gut symptoms spike.
These are different trials, so the percentages are not a fair head-to-head. More: semaglutide vs tirzepatide, Foundayo vs Wegovy pill and tirzepatide side effects.
Serious side effects to know about
These are less common, but the FDA labels for Wegovy, Zepbound, Foundayo and Ozempic warn about them. Get medical help if you notice the signs.
| Problem | Signs to watch for |
|---|---|
| Thyroid tumors (boxed warning) | A lump or swelling in your neck, trouble swallowing, a hoarse voice, or shortness of breath |
| Pancreatitis | Severe stomach pain that won't go away, sometimes spreading to your back, with or without vomiting |
| Gallbladder problems | Pain in your upper stomach, fever, yellow skin or eyes, or clay-colored stools |
| Kidney problems from fluid loss | Vomiting or diarrhea that leaves you dehydrated |
| Severe stomach reactions | Stomach problems that are severe or don't let up |
| Serious allergic reaction | Swelling of the face, lips, tongue or throat, trouble breathing, fast heartbeat, severe rash |
| Low blood sugar | Shakiness, sweating, confusion — higher risk if you also take insulin or certain diabetes pills |
| Eye problems in type 2 diabetes | Vision changes, if you have diabetic eye disease |
| Problems during surgery or sedation | Food can stay in your stomach longer. Tell any doctor before a procedure |
Extra label notes that matter for Hims options:
- Wegovy also warns about a higher resting heart rate.
- Zepbound can make birth control pills work less well. The label advises switching to a non-pill method, or adding a barrier method, for 4 weeks after you start and after each dose increase.
- Foundayo still carries the thyroid boxed warning and the MTC / MEN 2 rule, even though its label notes orforglipron did not cause rodent thyroid tumors the way some other GLP-1s did in animals.
GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.

What Hims says about side effects — and when to message
Hims does not publish a detailed side-effect rate list of its own. What it does say on its weight loss and FAQ pages (checked November 6, 2026):
- Side effect support is part of membership. Hims markets "side effect support from the start" and 24/7 Care Team messaging.
- Nausea early on is expected. Hims's FAQ says it's normal to feel some nausea in the first few weeks.
- Generic Zofran may be included. If a provider prescribes it, Hims says a generic for Zofran (ondansetron) is included at no extra cost to help with nausea. Zofran is FDA-approved for nausea from chemo, radiation and surgery. Using it for GLP-1 nausea is off-label, at the provider's discretion.
- Many side effects fade. Hims says some side effects are common when you start and usually fade as your body adjusts. If they're hard to manage or don't go away, a provider can help.
- Dose changes are provider-led. Hims says a provider can raise or lower your dose, or help you switch medicines, if that fits better.
Message your Hims Care Team in the app if you:
- Have nausea, vomiting, diarrhea or constipation that keeps going or gets worse
- Can't keep fluids down
- Feel dizzy, weak or dehydrated
- Aren't sure whether to take your next dose
- Notice any of the serious signs in the table above
Call 911 for trouble breathing, swelling of your face, lips, tongue or throat, severe chest pain, or any emergency. Severe stomach pain that won't go away also needs fast care — it can be a sign of pancreatitis.

Who should talk to a doctor before Hims?
Tell a clinician before starting if you've had thyroid cancer, pancreatitis, gallbladder disease, severe stomach or gut disease (including gastroparesis), kidney disease, diabetic eye disease, or depression or suicidal thoughts. Also tell them if you're pregnant, trying to get pregnant, or breastfeeding.
GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2. Hims — and every other GLP-1 program — is the wrong path if those apply.
Hims is also not a fit if you need insurance to pay for the medicine. Hims says insurance isn't required for its program, which usually means cash-pay. Details: does Hims take insurance? and does Hims have a membership fee?.
Considering starting a Hims GLP-1?
For many people, the side effects are uncomfortable but manageable, and they tend to ease after the first months — if you stick with the dose schedule your provider sets and message early when symptoms are hard to live with.

FAQ
What are the most common Hims weight loss side effects?
Nausea, diarrhea, constipation, vomiting and stomach pain. On the brand-name labels for Wegovy, Zepbound and Foundayo, nausea was the most common, at roughly 25% to 44% depending on the drug and dose. Hims itself highlights nausea in the first few weeks and offers Care Team messaging plus generic Zofran if prescribed.
How long do Hims side effects last?
Hims says many side effects fade as your body adjusts. The FDA labels show gut symptoms are highest during dose increases. On Zepbound, most people who stopped because of side effects did so in the first few months. If symptoms aren't getting better, message your Care Team.
Does Hims cause hair loss?
Hims doesn't call out hair loss on its main weight loss page. On the brand labels, hair loss was reported in about 3% (Wegovy), 4% to 5% (Zepbound) and 4% to 5% (Foundayo) of people, vs about 1% to 2% on placebo. On Zepbound and Foundayo, hair loss was linked to weight reduction and was more common in women.
Are Foundayo side effects worse than Wegovy?
Foundayo's label showed nausea in 26% to 35% of people; Wegovy injection showed 44%. Those are different trials, so it isn't a fair head-to-head. Foundayo's stop rate for side effects (8%) was a bit higher than Wegovy injection (6.8%). Ask your provider which fits you. More: Foundayo vs Wegovy pill.
Can Hims GLP-1s cause low blood sugar?
The labels say the risk goes up if you also take insulin or certain diabetes pills (like sulfonylureas). Tell your provider about every medicine you take. Ozempic is a diabetes drug, so blood-sugar monitoring matters even more if that's what you're prescribed.
When should I go to the ER on Hims?
Call 911 for trouble breathing, swelling of your face, lips, tongue or throat, severe chest pain, or any emergency. Severe stomach pain that won't go away also needs fast care, since it can signal pancreatitis. Don't wait on a chat reply for those.
Does Hims include medicine for nausea?
Hims says a generic for Zofran (ondansetron) is included at no extra cost if a provider prescribes it. Zofran is FDA-approved for nausea from chemo, radiation and surgery. Using it for GLP-1 nausea is off-label.
