Ro Zepbound Cost: What You'll Really Pay in 2026

By WPG Research Team · Last verified · Last updated

Zepbound on Ro costs $299 to $449 a month for the medicine, plus the Ro Body membership. Ro lists the Zepbound KwikPen at $299 for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg through 15 mg. The membership is $39 for the first month, then $74 to $149 a month depending on the plan. That puts most people at $448 to $598 a month on a monthly plan.

Below: the price at every dose, what the membership adds, the 45-day refill rule that can raise your price to $699, and how the insurance route works.

Weight Loss Provider Guide is an independent comparison resource for GLP-1 telehealth providers.

Bottom line: Ro charges the same Zepbound KwikPen prices as LillyDirect. What you pay extra is the Ro Body membership, which runs $853 to $1,678 in year one.

Best for: Cash payers who want brand-name Zepbound shipped to their door, plus insurance help, coaching, and one place for care.

Not best for: People who only want the lowest cash price on Zepbound and don't need a membership. Anyone pregnant or under 18. GLP-1 safety: do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.

What matters most: Your dose. The 7.5 mg to 15 mg price is $449 only if you refill on time. Miss the 45-day window and a 10 mg to 15 mg refill is $699.

How Ro charges for Zepbound

Ro bills you two separate ways:

  1. The Ro Body membership. This is Ro's fee for care. It covers your provider, insurance help, messaging, check-ins, and coaching.
  2. The medicine. This is the Zepbound itself. It is billed on top of the membership.

Ro says you're only charged the membership if you're eligible. If you're not eligible for a GLP-1, Ro says your $39 first-month fee is refunded.

Ro Body membership prices

PlanPrice per monthNotes
First month$39Refunded if you're not eligible, per Ro
12-month prepaid$74Ro says this saves $900 a year
6-month prepaid$89
3-month prepaid$99
Month to month$149

Ro says the membership includes access to FDA-approved GLP-1s, a dedicated insurance concierge, unlimited provider messaging, side effect and dose support, weight and dose tracking, and 1:1 health coaching. The medicine is not included.

What does Zepbound cost on Ro at each dose?

Ro sells the Zepbound KwikPen for cash. One KwikPen holds four weekly doses, so one pen lasts about a month.

DoseRo's listed priceRo's list price before the offer
2.5 mg$299/month—
5 mg$399/month—
7.5 mg$449/month with manufacturer offer$499
10 mg$449/month with manufacturer offer$699
12.5 mg$449/month with manufacturer offer$699
15 mg$449/month with manufacturer offer$699

Ro says treatment usually starts at 2.5 mg and goes up each month. So your first two months cost less. From the third month on, most people pay $449 for the medicine.

Ro's prices match Lilly's. Ro says its cash prices match LillyDirect, NovoCare and TrumpRx. Lilly's own Zepbound savings page lists the same KwikPen prices: $299, $399, and $449 with the offer at 7.5 mg and up.

The 45-day refill rule

This is the part that catches people.

The $449 price for 7.5 mg to 15 mg is a manufacturer offer from Lilly. To keep it, you must finish your next refill check-in within 45 days of your last delivery.

If you miss the 45 days, Ro says you'll pay full price for that refill:

  • $499 for 7.5 mg.
  • $699 for 10 mg, 12.5 mg, or 15 mg.

That's $250 more for one month at the higher doses. Ro says it sends reminders as the deadline gets close. Set your own reminder too.

How much is Zepbound on Ro for a full year?

Here's our math for 12 months on a typical step-up: 2.5 mg, then 5 mg, then 7.5 mg or higher for the rest of the year, with every refill on time.

CostMonthly membership12-month membership plan
Medicine (12 pens)$5,188$5,188
Membership$1,678 ($39 + 11 × $149)$853 ($39 + 11 × $74)
Year-one total$6,866$6,041
Average per monthAbout $572About $503

How we got the medicine total: $299 + $399 + 10 × $449 = $5,188.

A few things to know about this math:

  • It's our estimate, not Ro's quote. Ro does not publish a year-one total for Zepbound.
  • The annual plan math assumes the $74 rate starts after your $39 first month. Ro's pricing page doesn't spell out the timing. Confirm it at checkout.
  • A full 52 weeks needs 13 pens, since each pen is four weekly doses. A 13th pen adds about $449 plus one more membership month.
  • One late refill at 10 mg or higher adds $250.

What does the membership add? The medicine costs the same through Lilly. So Ro's real extra cost is the membership: $853 to $1,678 in year one. That's the price of Ro's care team, insurance help and coaching on top of the drug.

Does Ro sell Zepbound vials?

No, not on the pages we checked. Ro's Zepbound page lists two forms on November 3, 2026:

  • Zepbound KwikPen: cash pay only, shipped to you.
  • Zepbound autoinjector pen: Ro checks your insurance, or you can pay list price. You pick it up at your local pharmacy.

If you want vials, see our list of Zepbound vial providers online.

Ro Zepbound cost with insurance

Ro can try to get your insurance to cover the Zepbound autoinjector pen, the single-dose weekly pen. The KwikPen is cash only.

Ro says its insurance concierge will:

  • Check whether your plan covers the pen.
  • Handle the paperwork, including prior authorization.
  • File an appeal if you're denied.

How long it takes: Ro says the concierge process "typically takes between 1-3 weeks." Prior authorization alone usually takes about 1 to 2 weeks.

What you pay with insurance:

  • The membership still applies. Ro says, "Insurance does not cover the cost of the Ro Body membership."
  • Your copay depends on your plan. Lilly's savings card says people with commercial insurance that covers the pen may pay "as little as $25" for a month. The card has a $100-a-month and $1,300-a-year savings cap and ends December 31, 2026.
  • If your commercial plan doesn't cover the pen, Lilly's card says you may pay as low as $499 a month for the single-dose pen.

If your insurance says no, Ro says you can switch to a cash-pay option or cancel your membership.

On Medicare or TRICARE? Ro says you may still be eligible for some cash-pay options. Lilly's savings card is not for people on government health plans.

How fast can you start Zepbound on Ro?

Ro says:

  • You find out if you're eligible within 2 days.
  • If you pay cash, your KwikPen ships to you and you can take your first dose in less than a week.
  • If you use insurance, plan on 1 to 2 weeks, and you pick up the pen at your pharmacy.

Is Ro the cheapest way to get Zepbound?

Not for the medicine alone. Ro's KwikPen prices match LillyDirect, so the drug costs the same. Ro adds the membership.

Ro makes more sense if you want:

  • Insurance help. Ro's concierge handles prior authorization and appeals.
  • One care team for messaging, dose changes and side effects.
  • Coaching and tracking built in.
  • Other options if Zepbound doesn't work out. Ro also lists Wegovy, the Wegovy pill, Foundayo, Ozempic and Saxenda.
