FSA Document Request Checklist for GLP-1 Care
Use this before checkout. It helps you ask for the documents many FSA administrators require.
Do not rely on an “FSA accepted” badge alone. Your plan administrator applies your plan’s rules. Keep the provider’s written answer, your prescription, and every receipt.
The five receipt fields
- Patient name
- Provider or merchant name
- Date of service or purchase
- Detailed product or service description
- Out-of-pocket amount
Ask the provider before you pay
- Can the FSA card be used at checkout?
- If it declines, can I pay another way and get a reimbursement-ready itemized receipt?
- Will the receipt identify the actual medication or clinical service?
- Can my treating clinician provide a letter of medical necessity if my plan asks?
- Is there a fee for a corrected receipt or LMN?
- What is due today, when is the next charge, and what is the cancellation cutoff?
- Which medical group and pharmacy may handle my care?

Subject: Itemized receipt needed for FSA reimbursement
Hello,
Before I pay, please confirm that you can provide an itemized receipt showing:
• patient name
• provider or merchant name
• date of service or purchase
• the actual prescription medication or clinical service
• amount paid
Please also tell me how I will download or request it and whether any part of the charge, such as coaching, supplements, shipping, or membership access, appears as a separate line.

Subject: FSA letter of medical necessity request
Hello,
My FSA administrator may require a letter of medical necessity. If my treating clinician agrees that one is medically appropriate, can the clinician provide a signed letter that identifies my diagnosed condition, the prescribed treatment, why it is medically necessary rather than general wellness, and the expected treatment period if required by my plan?
Please tell me whether there is a fee and how long the request usually takes.
